Intake forms may be dropped off in person at reception. If you come by outside of regular clinic hours, forms or mail can simply be put through the mail slot downstairs, in a sealed addressed envelope. getting here Body of Knowledge Healing Arts is located on the North side of Eglinton Ave W at Old Park Rd. We are about 5 blocks west of Bathurst Street, and a few blocks east of the Allen Rd. and Eglinton West subway station. There is a bus stop right at our door, and parking is readily available on Eglinton, surrounding side streets, or at the public lot diagonally across from us, on the S-W corner of Eglinton & Glen Cedar Rd.
