Follow the steps below which outline the intake forms to print & fill out, and what to expect of your visits.

1. prior to your first visit

c. Also, please download and read (not necessary to print) our privacy code which outlines the clinic’s privacy policy, and our policies around the collection, use and disclosure of your personal information:

We ask that you fill out the intake packages as completely as possible, and bring them with you to your first visit.

2. your first visit (90 minutes)

Your first visit includes an initial consultation with an in-depth review of your health history, and your first acupuncture treatment.

3. subsequent visits (60-75 minutes)
