Follow the steps below which outline the intake forms to print & fill out, and what to expect of your visits.

1. prior to your first visit

We ask that you fill out the intake packages as completely as possible, and bring them with you to your first visit.

2. your first visit (75-90 minutes)

Your first visit includes an initial consultation with an in-depth review of your health concerns and goals. Once an assessment is made an individualized treatment plan is created and treatment can begin.

3. subsequent visits (30-60 minutes)

Follow-up visits are scheduled according to your needs and treatment plan.
