Follow the steps below which outline which intake forms to print & fill out, and what to expect of your visits.

1. prior to your first visit

We ask that you fill out the intake packages as completely as possible, and return them to us by mail, fax, or in person the week before your first visit. Please follow the link to our contact information contact information »

2. your first visit (60-75 minutes)

The initial consultation includes an in-depth review of your health concerns and goals, a screening urinalysis, and a new patient information folder. The purpose of this visit is primarily information gathering and assessment. Basic treatment guidelines may be presented.

3. your second visit (up to 60 minutes)

Your second visit is usually scheduled about a week after your initial consultation, and allows us to complete any outstanding assessment, perform a screening physical examination, and review any requested lab work. We will present you with a customized treatment plan, discuss your expectations, and set a course to reach your health goals.

4. follow-up visits (15-60 minutes)

Follow-up visits are scheduled according to your needs and treatment plan. They may be used to monitor your progress, refine your treatment plan, or provide in-clinic therapies. As issues are resolved, the focus of our visits will move towards maintenance and prevention.

The frequency of your visits will depend on the therapies we choose and the amount of support you require in achieving your health goals. Once balance is achieved, we suggest you visit at least seasonally, as part of your regular maintenance and prevention program.
