1. prior to your first visit
a. Please download & review the consent form & clinic policies package for either acupuncture or cosmetic acupuncture, below. You do NOT need to print this package. We will review it with you and have you sign it before a witness when you arrive prior to your first appointment.

b. Also, please download and read (not necessary to print) our privacy code which outlines the clinic’s privacy policy, and our policies around the collection, use and disclosure of your personal information. You must read the privacy policy in order to sign the consent forms and receive treatment.
c. Please select the appropriate intake form below. You will be taken to an electronic version of our intake form that you can fill out online and submit electronically via a secure server. Your information is safe. Please complete and submit your intake form the week prior to your initial consultation. If you prefer, you may download, print and fill out the intake form by hand, by downloading a printable version below:

PRINTABLE FORMS (alternate to electronic): If you choose to print your form, you must fill it out completely and return it to us the week before your initial consultation by fax, letter mail or in-person (in a sealed addressed envelope) at reception or through our mail slot at ground level.

2. your first visit (up to 90 minutes)
Your first visit includes an initial consultation with an in-depth review of your health history, assessment and your first acupuncture treatment.
3. subsequent visits
(60 minutes for facial rejuvenation treatments, 40 minutes for clear complexion treatments, 40 minutes for acupuncture with naturopathic support, 30 minutes for acupuncture only)
