Please read and complete the appropriate forms as instructed below. We ask that you complete and submit your intake form to us THE WEEK PRIOR to your appointment. This is mandatory, as your Naturopathic Doctor will review the information in your forms and prepare for your consultation prior to meeting with you.
a. Please download and read the consent form & clinic policies package below. There is no need to print it - we will review it with you and have you sign it before a witness when you come to your first appointment. To save time, please review these documents prior to arriving. Please be sure to come 15 minutes early to your first appointment to provide a sample for a screening urine test and to review and sign the consent forms.

c. Select the appropriate intake form below. Choose from: adult intake (18 yrs or older), teen intake (13-17 years) or pediatric intake (0-12 years).
For your convenience, our intake forms are now available as online questionnaires that can be sent to us electronically via a secure server. Your information is safe. We ask that you fill out the form as completely as possible, and submit it the week before your visit. If you prefer to download and print the forms to fill out by hand, they are available below – but you must return it to us by mail, fax, or in person (in a sealed addressed envelope) the week before your first visit.

d. Finally, regardless of age, we ask that you print a diet diary (below) for each new patient. Please fill it out in detail, and bring it with you to your first visit.
2. your first visit (90 minutes, plus arrive 15 minutes early)
We ask that you arrive 15 minutes early for your first appointment. This is so that we may conduct a screening urinalysis test and take care of some administrative details, including reviewing and signing our consent forms before a witness. Treatment cannot begin until the consent forms have been signed. To save time, please review our privacy policy, clinic policies and consent forms above, prior to your first visit.
Your initial consultation includes an in-depth review of your health concerns and goals. This visit focuses on assessment, including a screening physical exam and review of lab work. If you do not have copies of lab tests done in the past 6 months, we will fill out a form to request them from your MD. Additional or up-to-date diagnostic tests may be recommended, and introductory treatment guidelines are presented.
For children (up to 12) the initial consultation is 60-75 minutes in length, and we are usually able to provide both assessment and treatment in the first visit (older children or more involved cases may require a second visit of 30-60 minutes in length to complete any outstanding assessment or elaborate on a treatment plan - otherwise, we will simply follow-up as needed). Again, we ask that you come 15 minutes early to your child’s first visit.
3. your second visit (60 minutes; mandatory for adults and teens)
For adults and teens a second visit is scheduled 2-4 weeks later in which lab results are reviewed, an assessment is presented and treatment goals are discussed. You will receive a customized treatment plan with dietary, lifestyle and supportive therapeutic recommendations and your personal Patient Resources Binder. Your ND will review all information with you and answer any questions you may have about your treatment.
4. follow-up visits (15-60 minutes)
Follow-up visits are scheduled according to your needs and your ND’s recommendations. They may be used to monitor your progress and adherence, refine your treatment plan, or provide in-clinic therapies. As issues are resolved, the focus of our visits will move towards maintenance and prevention. Revisions and suggestions are made as necessary.
Typically follow-up visits are scheduled for 45 minutes, but if you only use 30 minutes you will be billed accordingly for time used. These visits will review current health concerns being addressed. If there is a new concern that arises, or it has been more than 6 months since your last visit, we recommend that you schedule an extended 60 minute follow-up to allow adequate time for assessment, education and treatment plan revisions. Additionally, if you prefer to budget for 15 or 30 minute follow-ups, please discuss it with your ND ahead of time to allow for appropriate scheduling and to ensure your time is used most efficiently.
The frequency of your visits will depend on the therapies we choose and the amount of support you require in achieving your health goals. Initially, it is typical to schedule visits every 4-8 weeks, or more frequently for acupuncture. Once balance is achieved, we suggest you visit at least seasonally (3-6 times per year), as part of your regular maintenance and prevention program.
