Please read and complete the appropriate forms as instructed below. We ask that you complete and submit your intake form to us AT LEAST WEEK PRIOR to your appointment. This is mandatory, as Dr. Baer will review the information in your forms and prepare a nutritional analysis and meal plan to present you with at your initial consultation.

There is no need to print it - we will review it with you and have you sign it before a witness when you come to your first appointment. To save time, please review these documents prior to arriving. Please be sure to come 5-10 minutes early to your first appointment to review and sign the consent forms.

c. Please submit the following intake form AT LEAST ONE WEEK PRIOR TO YOUR FIRST VISIT.
For your convenience, our intake forms are now available as online questionnaires that can be sent to us electronically via a secure server. Your information is safe. We ask that you fill out the form as completely as possible. If you prefer to download and print the form to fill out by hand, it is available below – but you must return it to us by mail, fax, or in person (in a sealed addressed envelope) at least one week before your first visit. 2. your first visit (45 minutes, plus arrive 5-10 minutes early)
We ask that you arrive 5-10 minutes early for your first appointment. This is so that we may take care of some administrative details, including reviewing and signing our consent forms before a witness. Treatment cannot begin until the consent forms have been signed. To save time, please review our privacy policy, clinic policies and consent forms above, prior to your first visit.
Your initial consultation includes an in-depth review of your health concerns nutritional needs and goals. A nutritional analysis outlining your current nutritional status will be presented – identifying areas in your diet that need attention. Baseline measurements (blood pressure, heart rate, waist and hip measurements, height & weight, BMI, BMR) will be recorded, and recent lab results will be requested and/or discussed as available. You will also receive a 7-day meal plan customized to your nutritional needs, lifestyle and reported preferences. Educational materials and supportive tools will be provided.
3. your second visit (30 minutes; within 1-4 weeks of your initial visit)
This second visit will be used to receive feedback from you and make any necessary adjustments to your plan. Measurements may be taken to assess progress, and further educational materials will be presented. Additional weekly meal plans may be purchased if desired. Tracking forms/diet diaries will be reviewed and discussed.
4. follow-up visits (15-30 minutes)
Follow-up visits are scheduled according to your needs and your ND’s recommendations. They may be used to monitor your progress and adherence, refine your treatment plan, or provide educational materials and support. Additional weekly meal plans may be purchased at any time.
For new patients, we recommend that you combine your nutritional consultations with our Roots of Health Program™ (designed to cover the fundamentals of a naturopathic approach to wellness and prevention, through the key areas of elimination & detoxification function, nutrition, healthy digestion, inflammation, healing & repair, and stress management), Heart Smarts Program™ (focusing on the treatment and prevention of cardiovascular disease and diabetes), or our Lighten Up Program™ (a weight management program prioritizing healthy and sustainable weight management over aesthetics or gimmicks).
